Stage 1 Application for Centre Approval

This questionnaire is the first stage in the EAQ Centre Approval process.  It is your opportunity to tell us about yourself and your premises, the facilities you have and the qualifications and experience of your staff. You do not need to have everything in place when you complete this first stage and you are not expected to know everything about running a centre or being an Equine Assisted Facilitator.  We run our own training programmes to help you with those things.  Once we have received your application we will contact you with details of the next stage and arrange for a member of our Approvals Team to visit you.
